Answer: Termination Care Management:  The relationship between the case manager and the client is based on the care requirements of the client. The process of termination of case management resulted in severance of relationship between the two. It is important to have an effective strategy to manage the termination of a case between the two smoothly. One of the responsibilities of the case manager is to ensure that the client is provided with appropriate services to achieve the goals which were set at the initial stage of case management. The objective of case management is to ensure that after the completion of treatment the client is emotionally, mentally and physically stable to take care of himself without any problem (Fairchild & Pierson, 2013). It is very important to effectively terminate a case to achieve that objective. A detailed discussion on the strategies for termination of case management is provided below to help the readers to understand the effective method to terminate a case.    Understanding The Expectation Of The Client: Firstly, the case manager must understand the actual expectation of the client from the treatment. Understating the expectation of the client would help the case manager to provide appropriate treatment to the former. Maintaining Effective Communication With The Client: Maintaining an effective communication line with the client will be helpful in the termination process. In order to communicate various matters including the termination process it is essential to have an effective communication mechanism between the two (Fairchild & Pierson, 2013). Informing The Client About The Termination Process And Length Of The Treatment Plan: The case manager must inform the client about the length of the treatment plan to ensure that the later has fair idea about the possible termination strategy. The client shall be aware of the termination process since the beginning to understand the treatment process as well as the process of termination since the beginning of the treatment plan (Griffiths, 2010). Complying With The Terms And Conditions Of The Treatment Plan: The case manager must comply with the terms and conditions of the treatment plan to satisfy his client. It would be an ideal situation for the case manager to terminate the case subsequent to the achievement of the objectives and goals of treatment determined at the beginning (Minor & Minor, 2016). Evaluation Of The Treatment: An evaluation of the treatment/ treatments provided to the client over a particular period of time shall be helpful in understanding whether the case manager has provided the treatment and other services as per the expectation of the former. In case the client is not satisfied with the services and treatments of the case manager then generally the termination process stretches little longer. Ability Of The Client To Take Full Responsibility Of Himself: Evaluation of each situation carefully is the responsibility of a case manager. It is expected that the client will be able to take full responsibility of complying with the requirements of independent care at the time of termination of the case. Thus, until unless the client is capable of taking independent care of himself the case manager should not proceed to terminate the case. Apart from the above steps it is important for the case manager to assess the practical situation of each client carefully. The process of termination shall start only after making it sure that the client is ready to take care of himself independently (Shannon, Dextrom & Fuhrhop, 2016). Independent Care And Continued Client Growth:  Independent care will obviously be helpful to the growth of the client. A client will continue to grow significantly with independent care. The following are the main reasons behind continued growth of a client under independent care. Specific care of the client is taken in independent care: It is possible to take specific care of a client under independent care as opposed to any other form of care. The progress of a client is independently monitored: It is possible to independently monitor the progress of a client. As a result necessary adjustments to the treatments can be made to improve his progress (Minor & Minor, 2017). Making changes to the treatment: In independent care it is feasible to make changes to the treatment plan as and when required by the specific client.  In team sessions it is not possible to make changes to the treatment plan easily. In fact a long procedure must be followed before any changes can be made to the treatment plan. Thus, even if an individual requires different treatment in team sessions and treatment it is not possible to provide such treatments and care to individuals. Thus, the independent care will obviously be of great help to the growth of a client.
