According to the IOM report (2003), all health clinicians should possess 5 core competencies: The ability to provide patient-centered care Be a part of an interdisciplinary team Utilize evidence-based practice Understand and be able to apply quality improvement processes Utilize informatics I agree that the Institute of Medicine is attempting to incorporate what would be considered a complete package of caring for all patients in a panel and focuses well on population health. If each profession utilizes evidence-based care where the focus remains on keeping the patient front and center, then patients are likely to have the best care from all professions. Additionally, when professions are able to work with an interdisciplinary team and understand what each profession brings to the table, there is a better ability to identify and implement quality improvement processes and streamline care. I think that working in an interdisciplinary is a little easier done in the inpatient setting especially since the implementation of the care coordinators and discharge planning nurses. These nursing roles have helped significantly with the transition of care from the hospital to home, making appointments, setting up home care and equipment, placement in nursing homes or assisted living. There is also more availability of needed disciplines in the hospital setting where the disciplines come to the patient compared to a primary care clinic where the patient may need to go to multiple appointments. However, clinics that have begun working as a Medical Home have made great efforts to move toward more of the interdisciplinary team approach. I also believe that informatics is the wave of the future for healthcare management. However, I think that the utilization of informatics must not hinder patient care, but rather complement it. So much of informatics is tied to what is documented or not documented. As I mentioned earlier in the semester, I previously worked as an EHR Specialist and had the opportunity to be on implementation teams for EPIC. I also was able to be a part of the development of reports and work with report writers to develop reports that tell us about the patients that we have cared for or are currently caring for. As clinicians, there is an important concept of documentation that not everyone understands and that is about utilizing discrete data fields, when possible, as this is how we are able to capture data that make these reports valuable. However, this creates a quandary because documentation into discrete data fields does not necessarily provide a streamlined process. Every ICD-10 code can help develop pools of patient populations that can help to trigger best practice advisories. However, providers must be diligent in their documentation to assure that the coding is correct and documentation regarding measures are documented in the correct field that is pulled for meeting measures. Answer: The core competencies that every clinical professionals should possess are- Provision of a patient centred care Ability to work with the interdisciplinary teams Using evidence based researches throughout the clinical practice Applying the quality improvement strategies Ability to use information technology in health care (Health Professions Education: A Bridge to Quality., 2003). The IOM report (2003) has rightfully pointed out the core competencies that are required to be infused in each of the health professionals. The increasing needs of the health care consumers have largely expanded the role of the clinicians and they have push their conventional limits to provide a patient centred care, that is provision of care that is responsive to individual preferences of the patients. Nowadays, nurses have to work in frontline with the physicians in the critical cases where they have to apply their clinical reasoning and analytical skills (Meretoja & Koponen, 2012). Evidence based research allows the nurses to remain updated with the recent researches, that can be implemented in to their current practice. Nurses often have to work as members of the multidisciplinary teams and take part in shared decision making. The nurses often act as key link in the communication network within the multidisciplinary team (Meretoja & Koponen, 2012). I also agree to the fact that a nurse should have a substantial knowledge about information technology. If, not they should be given robust training about the use of health care informatics like electronic medical records or bar code reading. For the smooth functioning of health care organisations health informatics should be incorporated in the billing system, transition and the discharge systems.
